| To assist the ADON in the daily activities occurring on the unit by performing clerical and support duties and ensuring all chart work is completed for all individuals in residence. |
Duties: |
1.0 Assists in assuring accurate and timely completion of forms and documents for all shifts.
1.1 Performs all individual record activities with close attention to detail and accuracy.
1.2 Checks to ensure that nursing notes are completed each shift.
1.3 Checks to ensure that treatment plan is completed and signed by treatment team and individual.
1.4 Checks to ensure that psychosocial is completed and signed within 72 hours of individual’s admission if needed.
1.5 Checks to ensure that doctor's orders are signed off by RN and reviewed every 24 hours.
1.6 Maintains individual’s charts in appropriate organizational and sequential order.
1.7 Automatically calls for and obtains medical records for individuals upon their admission.
1.8 Readily performs clerical functions related to the admission and discharge of individuals, i.e., transcription of orders, etc.
2.0 Assists in the coordination of individual discharges and transfers.
2.1 Checks to ensure all documents requiring individual and staff signatures have been properly completed.
2.2 Accurately prepare SISAR forms upon discharge and routes to MIS.
2.3 Individual charts are photocopied, as required.
2.4 Prepares all charts of discharged individual within the same day that the individual was discharged.
2.5 Individuals charts are forwarded to the Medical Records component upon completion of all noted deficiencies, i.e., discharge summary, etc.
3.0 Answers telephone, takes messages and communicates messages to the appropriate person.
3.1 Receives, places and transfers calls using appropriate telephone etiquette; handles telephone information requests with courtesy, accuracy and respect for individual confidentiality.
3.2 Demonstrates good judgment in screening and/or routing calls.
3.3 Takes accurate telephone messages and relays appropriate information in a timely manner.
3.4 Demonstrates effective communicative ability; conveys information appropriately.
4.0 Assists clinical staff in the observation, supervision and monitoring of individuals in the milieu.
4.1 Assists in the orientation of new individuals to the program.
4.2 Assists individuals in increasing ADL skills using hands-on techniques as needed.
4.3 All individualson special precautions (close observation) are monitored according to facility protocol and procedure.
4.4 Recognizes signs of aggressive behavior and responds promptly to crisis situations.
4.5 Appropriately utilizes verbal and physical intervention techniques (NAPPI) to redirect and prompt individuals to safe and appropriate behavior.
5.0 Models respectful interactions with youths and guardians that demonstrate:
5.1 Age appropriate language, content and basic knowledge of developmental stages
6.0 Performs other duties as requested.
6.1 Miscellaneous duties are performed according to instruction.

Qualifications: |
| High School Diploma or equivalent, plus three years medical/clerical experience. Ability to type 50 WPM accurately; computer literate. |
