Description: |
| Candidate will provide goal-oriented and individualized support for adolescents through assessment, planning, linkage, advocacy, coordination and monitoring of service activities for our Community Based Intervention Services Program (CBISP). |
Duties: |
1. Performs functions involved with assessment, including initial contact with the youth, facility personnel, and parent/legal guardian regarding the youth’s needs, presenting problems, and any other related issues when referral is received from the facility. Referrals are received at least 90 days prior to discharge.
2. Completes assessment of each youth to include psychosocial history, mental status, behavior, family dynamics, educational/vocational performance, medical needs, and legal status. This assessment is used to identify each youth and family’s strengths as well as barriers toward successful reintegration back into the home community.
3. Develops a Performance Plan (PP) that ensures access to services identified in the assessment as contributing to the youth’s success in reintegration to his/her community. The PP includes goals relative to the medical, educational/vocational, independent living (if applicable), mental health and substance abuse issues for the youth.
4. Participates in discharge staffing meetings to ensure that services in the PP respond to the strengths and needs of the youth.
5. Participates in treatment team reviews with the youth, therapist, and other members of the treatment team.
6. Monitors youth’s progress and needs upon discharge through regular discussion with the youth, therapist, designated school or vocational personnel, and other facility staff members.
7. Makes regular contact with parent or legal guardian of youth for assessment purposes and to explain the services of case management in preparation for youth’s discharge from the facility.
8. Documents case management activities in chronological notes according to Foundation Wide policies and procedures. This documentation shall consistently refer to the goals and objectives specified in the youth’s PP and the progress or lack of progress toward these goals.
9. Provides other written progress reports to facility as requested.
10. Maintains each youth’s file on assigned caseload in manner consistent Foundation Wide policies and procedures and in accordance with contract requirements.
11. Collaborates with supervisor to develop and maintain a schedule of service provisions consistent with weekly benchmark expectations as established in the program’s strategic plan. Meets with supervisor regularly to ensure schedule is efficient and effective.
12. Ensures that all assessments, PP reviews, transition plans and discharge summaries are reviewed and signed by the supervisor.
13. Stays abreast of current information in the field and resources available through training attendance as recommended by the supervisor.
14. Follows all Foundation Wide Policies & Procedures including Protected Health Information (PHI) as required by Federal & state laws.

Qualifications: |
| BA/BS degree in social work or any other human services field with one year experience working with adolescents. |
