Sr. Mental Health Practitioner

Description:

To provide treatment and service coordination for clients, and to provide individual, family and group therapy including psychosocial assessment. To coordinate and plan effective discharge and follow-up care.

Duties:

1.0 Conducts psychosocial evaluations for new and existing clients.
1.1 Psychosocial evaluations are performed in accordance with the Agency’s protocol for conducting psychosocial evaluations and its procedure relative to frequency and timeliness for the occurrence of this activity.
1.2 Verbalizes and understands Agency admission criteria and procedures, Baker Act criteria and Client Rights.

2.0 Develops and maintains a written treatment plan for each client.
2.1 Written treatment plans are developed with as much participation for the client and family members as possible and reasonable.
2.2 Treatment Plans are maintained in accordance with the Agency’s protocol and procedures for frequency and timeliness for completion of this document.
2.3 Treatment plans are appropriately signed and properly credentialed.


3.0 Conducts individual and multi-client therapy activities.
3.1 Individual and multi-client counseling activities are performed in accordance with the Agency’s standards for the use of various counseling modalities, as well as frequency and length of counseling sessions.

4.0 Leads and participates with team members in appropriate treatment interventions.
4.1 Conducts education, lecture and group discussions with clients and family members.
4.2 Coordinates written assignments and individual goal setting activities with assigned clients.
4.3 Coordinates and participates in weekly treatment team meetings.
4.4 Appropriately utilizes verbal and physical intervention techniques (NAPPI) to re-direct and prompt clients to safe and appropriate behavior.
4.5 Consistently motivates MHTs in a positive manner; develops teamwork in accepting and practicing the policies and procedures of the Agency.
4.6 Provides direction as necessary; always leads by professional example.
4.7 Actively engages families in treatment process.

5.0 Ensures that appropriate discharge planning and aftercare services are provided for assigned clients.
5.1 Discharge Plan and aftercare services are provided for assigned clients.
5.2 Conducts sessions with families and other significant persons to encourage support and involvement in client’s discharge and aftercare plan.
5.3 Follow up client discharge to ensure safety/reviews with Team as appropriate.

6.0 Prepares written documentation for each service conducted.
6.1 Written documentation for each service conducted is performed in accordance with the Agency’s standards for service documentation in the client’s medical record.
6.2 Medical Records accurately reflect the nature of service activity provided, timeliness of services and outcome.

7.0 Performs other duties as requested.
7.1 Miscellaneous duties are performed according to instruction.

Qualifications:

Masters Degree in Social Work or Counseling is preferred with two years experience working with children/adolescents
