Description: | The Family Service Coordinator (FSC) is responsible for the coordination, facilitation and implementation of early intervention services for eligible children and their families in the Early Steps Program.
Bilingual Preferred | Duties: | Specific job functions include, but are not limited to:
(1) Completion of first contacts with families upon referral
(2) Development and implementation of the Individual Family Support Plan (IFSP) within a natural environment primary service provider model
(3)Transition to other services and ensuring procedural safeguards under the Individual with Disabilities Education Act (IDEA) Part C
(4) Attend initial eligibility evaluations | Qualifications: | Bachelor's degree in a human service field such as social work, education, psychology or a related field; and one year of related experience and case management with infants and toddlers or children and families; preference with special needs population.
